What trends are you seeing with care coordination tools this year, and what tools will be instrumental this year in laying the groundwork for ACOs?
Each ACO has taken a different approach to their work. Electronic health records (EHRs) will play an important role for each, and the Pioneer ACO Model requires each ACO to obtain meaningful use in their systems. Many are establishing new analytical systems that help them make improvements to the care that they provide to patients. We’ve also seen new patient portals that help engage patients in their care through better access, and communicate with their care managers. We’re excited to see what the ACOs come up with, and how we can find new creative ways to explore care coordination.
