Well I was sure that there would be major adjustments to be made when I left the military after 21 years. I guess the biggest cultural change came down to the uniform. In the military you always know who is in charge by all the “ornaments” in the uniform. The more the colors and “bling,” the higher up the food chain they are, especially the Army-they are all about the bling!

Reach back to all your old quality training, Deming, Toyota, whatever and at the core it’s the same message; your front line workers really know what’s going on and what workarounds they are using because the existing system is so bad. They are also the first ones to create new workarounds because the brand new system now takes five mouse clicks to do what the old one did in two.

I find it ironic that after investing money and effort in an EMR implementation, many organizations are still handing out paper forms to collect history, insurance updates and HIPAA acknowledgements.

I don’t understand why application vendors place little importance on reporting requirements. I get the fact that customers like to customize their reports, but why not make that an inherent feature? Maybe that is why the market is constantly creating so many 3rd party Business Intelligence (BI) vendors. But what about Clinical Intelligence (CI) requirements?

During one of the HIMSS Roundtable discussions, the topic of recruiting IT talent from other industries and how to migrate them to Healthcare IT kept coming up. The vast majority of CIO’s and Directors all agreed that we must tap into this talent pool, but these candidates lacked needed skills necessary to relate to clinicians and they did not understand the unique challenges of Healthcare.

Many large organizations have embraced the concept of a centralized approach to Project Management, but the downside is that staff become so engrained with bureaucracy that they fail to be nimble. There is a fine line between having resources that assist with project management and having someone that checks blocks and has an opinion.
