In the opening keynote address to the HIMSS Virtual Conference and Expo on June 6, the CEO of MedVirginia, one of the pioneering health information exchanges in the country, offered up some reflections on HIE evolution and future direction. Among other things, Michael Matthews said that the focus on return on investment is holding HIEs to a higher financial standard than other aspects of treatment.

Hospitals and health systems are fueling the growth of private health information exchanges (HIEs) to strengthen care coordination among their employed and affiliated physicians. One example of this surge in private activity is a grassroots, community-wide HIE cultivated by Huntington Hospital, a 626-bed regional medical center located in Pasadena, Calif. Huntington has used its mantra “Right Care, Right Place, Right Time,” to facilitate coordination across the care continuum and link its large population of 3,200 community physicians (2,300 physicians of whom are not on the medical staff).

After a period of wait-and-see in the health information exchange (HIE) market when the American Reinvestment and Recovery/Health Information Technology for Economic and Clinical Health (ARRA-HITECH) Act initially passed in 2009, meaningful use has since been pushing providers to adopt the EHR technology necessary to move forward to launch HIEs. Hospitals and health systems, as well as payers, are fueling HIE growth, building information backbones necessary to support care coordination and accountable care organization (ACO) development.

CIOs at a HIMSS12 breakfast, sponsored by Siemens Healthcare (Malvern, Pa.) on Tuesday Feb. 21 spoke about how their organizations were developing private health information exchanges (HIEs) to link their physician practices and ultimately drive sustainability and competitive advantage. John Glaser, Ph.D., CEO, Siemens Healthcare noted there were two models, ownership versus contractual, for healthcare integration among hospitals and community physicians.

Attempting to bridge the divide between primary and specialty care, Michigan Health Connect (MHC), a Grand Rapids-based health information exchange (HIE) founded by leading Michigan health systems, developed a free eReferrals app for community physicians. The value and ease of use of that app spurred rapid adoption and made this project well-qualified for the Healthcare Informatics Innovator Award. What really is impressive is how this app can not only be used to transform communication between primary and specialty care, but how it can be applied to other areas of healthcare.

The New York eHealth Collaborative has signed a Memorandum of Understanding (MOU) with the Informatics Corporation of American (Nashville, Tenn.) to join the public-private multi-state working group comprised of Electronic Health Record (EHR) vendors, Health Information Exchange (HIE) vendors and states. The group is looking to create common standards, and is committed to develop the technical specifications to enable “plug and play” connectivity between EHRs and HIEs.

In recent years, the number of live private health information exchanges (HIEs) has been mushrooming and outpacing that of the public sector. In both sectors, organizations must reach a consensus among stakeholders over a variety of issues like governance, data sharing agreements, and so forth. A relatively new HIE, Carolina eHealth Alliance (CeHA), a Charleston, S.C.-based private HIE, is a good example of how competing systems can collaborate on patient care. With that said, CeHA has had to make some concessions in the face of competition.

The Orem, Utah-based research firm, KLAS has released a report stating that while providers are weighing various priorities including meaningful use, ICD-10, RAC audits, and accountable care models, HIEs are still of high interest. In the report, HIE Perception 2011: Public or Private?, KLAS interviewed 96 healthcare executives to uncover which HIE vendors were selected in 2011 and why.

Exciting things are happening in Michigan, where a broad, regional health information exchange (HIE) has been steadily evolving forward. Michigan Health Connect was officially incorporated in March 2010, and has been eliciting interest across disparate parts of the state. Doug Dietzman is executive director of the Grand Rapids-based information exchange organization. He spoke recently with HCI Editor-in-Chief Mark Hagland regarding Michigan Health Connect’s development and progress.
